WII GIVEAWAY RULES, TERMS, AND CONDITIONS


HealthCentric, LLC's Wii Giveaway ("Giveaway") is open to currently active, inactive, and prospective HealthCentric healthcare workers ("Field Staff"). In order to be eligible to participate, entrants mustbe a licensed healthcare professional and complete all blanks in the entry form, in addition to agreeing to the Rules, Terms, and Conditions set forth below.

In order to receive an entry, the formal online referral form must be completed. Any referrals made by Field Staff without completion of the form will not receive an entry.


General Terms and Conditions

All participants must be at least twenty-one years of age to enter. Participants represent that they are of sufficient legal age to use the registration site and to create binding legal obligations for any liability a participant may incur as a result of the Giveaway. Participant warrants that all information supplied by participant in regards to the Giveaway is true and correct.

The Giveaway ends on October 15, 2008, at 11:59 p.m. EDT. The winner will be announced on November 1, 2008. The contest winner will be determined by a drawing to be held on October 15,2008 in Southlake, Texas. The winner will receive a Wii and will be notified by a HealthCentric representative via the contact information provided during registration. Should a participant decline the prize or fail to contact a HealthCentric representative within thirty (30) days of notification, a new winner will be drawn.

All participants must be in good standing with HealthCentric at the time their trip is claimed. Should any participant be terminated for cause before the prize is claimed, he or she forfeits his or her prize and a new winner will be designated.

The prize must be claimed within a one year time period commencing on November 1, 2008. These Terms and Conditions may be amended from time to time and this giveaway is void where prohibited.


    In the event that any of the provisions of this Agreement are found by a judicial court to be unenforceable, the remaining provisions will, at HealthCentric's discretion, remain enforceable.

   HealthCentric and participant consent and agree that any legal action or proceedings with respect to this Agreement be brought in any of the Federal or State courts having subject matter jurisdiction in Tarrant County, State of Texas and irrevocably waive, to the fullest extent permitted by law, any objection which they may have to the laying of venue of any suit, action, or proceedings with respect to such documents brought in any such court, and further irrevocably waive, to the fullest extent permitted by law, any claim that any such suit, action, or proceedings brought in any such court has been brought in an inconvenient forum.
